Completing your Pre-Admission Survey:

Lafayette Surgery Center offers patients the convenience and privacy of a secure , online pre-admission process.

You will receive a text message containing a link to our Pre-Admission Assessment prior to your surgery date. You will be asked about your health history , medications and previous surgeries. It is important to complete as soon as possible so that your medical team will have time to review your information prior to your visit.  If you do not have internet access, please call our office and a nurse will collect all necessary medical information.

We will call you to go over dietary restrictions, answer any questions and to give you the check-in time for your procedure.
